The Tour Guy runs this one from Piazzale Flaminio, just outside Piazza del Popolo, at 7:30. The coach stops below Bagnoregio and the shuttle runs you to the Belvedere, where the bridge starts; the Civita ticket and the shuttle are both paid. You get about an hour and three quarters in all. Then the coach heads into Tuscany to a family-run winery near Montepulciano, for cured meats and pecorino, a pici-making demonstration, a pasta lunch with three wines, and cantucci with Vin Santo. The last stop is about an hour in Pienza, the Renaissance town over the Val d'Orcia.
At about $99 it undercuts the City Wonders trip and covers more. The operator's own page lists the Civita entrance, shuttle, winery visit, lunch and tastings as included; only tips are extra. What it lacks is Orvieto, so pick by your second stop: Tuscan scenery here, Orvieto's cathedral on the City Wonders trip.
Two honest warnings from guests. Even with the shuttle, the walk to Civita is strenuous: the bridge is about 300 metres and climbs steeply at the end. And the day can run late; the operator says it returns in the early evening, but traffic on the A1 decides that.
